WELLNESS Monthly Membership

$99 one time initiation fee
Benefits:
Member can choose one of the following services every month*:
●One Basic IV Infusion ~ $200 value
●One OMT Therapy (not including initial assessment) ~ $200 value
●One Carbon Laser Peel ~ $250 value
●One No Downtime Microneedeling Nano Fusion ~ $275 value
$50 OFF 1 area of Botox/Xeomin/Dysport or Dermalfillers
*Additional services are available at additional charges for 15% off and all products are 15% off*

Rules:
●Membership has a minimum of 6 months contract
●Cancelable any time after 6 months of the membership
●Cancelation within 6 months of the membership will result in a termination fee of $300 or will be required to pay for the remaining months of the contract
●30 day cancellation notice is required with email or written notices to the spa.
●Membership dues will be recurred and charged to your designated credit card on the 1st of every month.
●Membership will auto-renew every month with your signed consent.
●Services can be redeemed by friends and family upon client’s email request, but may not be issued as gift cards.
●Services can be rolled over to future months if not rendered, up to a year.
●Member can freeze the membership up to 3 months (no recurring charges during freeze term). Membership may be unfrozen at anytime with email or written notices to the spa. Without reinstating notice at the end of freeze term, membership will be considered cancelled and inactive. New activation fee will be applied as a new membership is activated.
●A valid ID and credit card is required to enroll in Beautyfluff's VIP monthly membership.
●Beautyfluff reserves the right to adjust membership pricing or membership services with a 30-days electronic notice.
●Spa membership dues do not include gratuities. Members are encouraged to tip on the full price of services
●Monthly memberships may not be used in combination with any other promotional offers or packages, discount coupons or gift certificates.
● Monthly memberships have no cash value, but can be used as store credit towards other services.
●Member must follow our Cancellation Policy for our appointment reservations. Please notify us at least 24 hours prior to the scheduled appointment to avoid being charged a No-Show Fee of $50.00.
●Beautyfluff reserves the right to cancel any membership at any time.
